Pharmacist – Poison Center
Description
Purpose:
Provide safe, effective, and cost-efficient medication therapy by supporting medication-use systems, optimizing patient care, preventing medication-related issues, ensuring regulatory compliance, and serving as an educational and clinical resource for healthcare teams, students, and residents.
Responsibilities:
+ Review, interpret, verify, and dispense medication orders and prescriptions in compliance with state, federal, and organizational regulations.
+ Assess patient medication profiles for allergies, interactions, duplications, and appropriateness of therapy, while identifying and reporting adverse drug reactions.
+ Promote safe and cost-effective medication use through formulary management, therapeutic interchange protocols, and evidence-based drug therapy recommendations.
+ Collaborate with physicians, nurses, and healthcare teams to clarify orders, resolve medication-related concerns, and optimize patient outcomes.
+ Prepare and dispense medications, including sterile and IV admixtures, using appropriate technology and aseptic techniques.
+ Support pharmacy operations by coordinating daily workflow and supervising pharmacy technicians, interns, aides, and other support personnel.
+ Serve as a drug information resource and provide education, training, and preceptorship to healthcare professionals, pharmacy students, and residents.
+ Graduate of an ACPE accredited College of Pharmacy
+ B.S in Pharmacy or Doctor of Pharmacy (PharmD) degree
+ Current licensure as a Pharmacist in practicing state
+ No previous experience required; pharmacist experience preferred.Licensure, Certifications, and Clearances:
