Job Purpose/Summary

 

The Outpatient Nurse provides nursing services to consumers and nursing support to medical prescribers in the outpatient setting. The incumbent utilizes medical and behavioral health skills to assist with the removal of medically related barriers to progress, as well as a professional nursing process to provide a holistic approach to nursing interventions. The position promotes mental health and physical well-being throughout treatment and crisis intervention. The Outpatient Nurse utilizes technical nursing skills to administer, dispense, and safe guard medications stored at the various agency centers. The incumbent performs nursing duties in accordance with standard agency practices and as per nursing license guidelines.

 

Minimum Qualifications

 

Education: Nursing license by the Commonwealth of Pennsylvania (LPN).

 

Experience:

  • Minimum of 6 months of experience working in a psychiatric setting preferred.
  • Experience with child, adolescent, and adult psychiatric nursing and/or substance abuse preferred.

 

Licenses, Certifications & Clearances:  

  • Act 33/34 Clearances
  • FBI Clearance
  • Valid State of PA Class 1 Drivers License

  • Knowledge of federal and state regulations and requirements relative to psychiatric/behavioral health nursing.
  • Critical observation, assessment, and reporting skills.
  • Knowledge of available community resources that may be needed as additional supports.
  • Knowledge of Microsoft Office and electronic medical record (EMR) keeping.
  • Ability to travel locally in between work sites.

 

 

Essential Duties & Responsibilities

 

  • Conducts mental health and physical health assessments, as needed and according to the ANA standards of nursing practice, providing appropriate follow-up.
  • Provides consultation/education to consumers, family members, and other support staff, as needed, via phone or in person.
  • Develops and maintains a treatment plan, which reflects consumer needs and goals. Reviews and updates the treatment plan with multidisciplinary treatment as per department regulations.
  • Prepares and administers medications as ordered by the agency MD and as per agency policies and nursing license regulations. Assists in safeguarding medications stored on agency premises in accordance with agency policies.
  • Assists medical prescribers with ordering any diagnostic testing and in assessing the need for medication refills and any potential adverse reactions.
  • Monitors laboratory results. Informs prescriber of any abnormal lab results or health-related issues. Recommends medical follow-up when appropriate. Assists consumers with arrangements for medical follow-up when necessary.
  • Completes any needed prior authorizations for consumers to obtain prescription medications and/or treatments.
  • Supports the agency in the development, implementation, and interventions of the Medication Assisted Treatment Program.
  • Acts as the consultant between behavioral health and physical health providers to improve coordination of care.
  • Assesses crises and responds to agency crisis codes. Assists in coordinating an appropriate plan of action.
  • Adheres to all privacy regulations and laws.
  • Acts in compliance with nursing licensure regulations and requirements.
  • Displays a working knowledge of agency and community resources. Refers consumers needing other services. Contacts collateral resources, such as physicians, other agencies, and supportive services. Collaborates with pharmacies, physicians, laboratories, service providers, schools, etc.
  • Maintains clinical records. Documents contacts with consumers and collaterals. Updates records/forms, as needed, and reviews ECR/charts for compliance with county and state regulations.
  • Completes all assigned trainings and stays up-to-date with current safety standards. Employs “universal blood and body fluid precautions” as recommended by the Center for Disease Control (CDC) when administering medications or providing direct care. Maintains clinical competence by utilizing supervision, current literature, consultation, and staff development opportunities.
  • Provides coverage for all nursing services as directed by the Supervisor – Nursing.
  • Acts as a consultant for other departments in regard to medical issues within the realm of nursing education.
  • Adheres to PA regulations and the requirements set forth by the federal agencies of OSHA and the Department of Transportation governing the management of infectious, chemotherapeutic, and pharmaceutical waste.

Key Requirements

$5,000 HIRING BONUS paid in installments: $1,500 at 90 days, $1,500 at 6 months, and $2,000 at 1 year!!

Pay: $23.00 per hour
